Introduction-

In this, it is based on the care plan that will focus on the health assessment approach. This research is based on the person - centered care along with some practices that are best by nature and as per the professional standards. It is also based on the safety and quality of the nursing. There is one case study discussed under this-

A 68 years old indigenous man Jiemba Button is suffering from a disease of Diabetes 2 about 10 months ago. He was admitted yesterday at Sunshine bay where he is respiring aged care plan (J Biddix, 2004).

Jimmy is living with his daughter Kierra, her husband Adam and grandson Monti. Grandson of Jimmy is at Lady Ciltro hospital undergoing cardio - vascular surgery because of a genetic heart condition. Both husband and wife are staying at Brisbane with a friend. Jimmy is having a deficit on the right side and a hanging on the right arm. He refused some assistance. He allowed assistance with dressing and cutting up of food. His BP is 189/96, HR 99, RR14, SP02 97% and Temp 37. Medication of Jimmy includes Metformin, Glipizide, Clopidogrel 75 mg and Atorvastatin 20 mg (L Irvine, 2016).

Diabetes is a very chronic disease where there is insufficient production of insulin in the pancreas or the failure of the body to use insulin effectively. Thus, in this case, nursing management will include the normalization of blood glucose with the decrease in complications with the help of replacement of insulin, good diet and routine exercise. It is necessary that the nurse should emphasize on the proper treatment program. Teaching need to be tailored based on the needs of the patients, stage of development and their abilities. It is necessary to stress more on the effective blood sugar control in order to live long term good health.(Barbara S. 2007).

- There is a deficiency of knowledge.

- There are more chances of infection.

- There is more chances of disturbance of sensory perception.

- There can be risk associated with injury.

- There can be chances of deficiency in the fluid volume.

- There are more chances of Fatigue.

Thus, there is actually a case that is considered by this assessment. The case of Jiemba button who is suffering from Diabetes 2. Thus, in this case, He is the indigenous man who is suffering from the Diabetes 2. Where he has to rely completely on medication and insulin. He needs to be injected by Insulin every day, just to bring his diabetes under control. This can be possible with the help of proper nursing. He got admitted to the hospital, because, suddenly the level of his diabetes rose up. Thus, he was in need of proper care at nursing to bring his diabetes under control. He immediately got admitted to the hospital. With proper medication, now his diabetes is under control.

Jimmy is suffering from slurred speech with an unsteady gait where, his right side is having some kind of deficit which needs to be cured. Proper treatment needs to be provided to Jimmy as he is not associating with the staff for treatment. He needs to associate with the staff in order to recover immediately. He is expecting immediate discharge which is not possible because of, nonassociation with the treatment. His medications are given which are closely associated with the Diabetes 2 (P Dunaway, 1987). His BP is too high. Thus, proper care needs to be taken for the same.

Explanation-

One can see the signs of hyperglycemia

If the amount of insulin in the blood glucose is inadequate, it will amount to hyperglycemia. This may result into excessive hunger, thirst and may increase the chances of urination. Such patient may suffer from blurred vision and fatigue.

Check the blood sugar level at bed time and before meal.

Thus, the range of blood glucose level should be between 140 to 180 mg/dl.

Proper monitoring of HbA1c- hemoglobin (glycosylated)

It desirable level should be between 6.5 to 7%

There can be signs of anxiety, tremors as well as slurring of speech. 50% dextrose need to be used to treat hypoglycemia

For this, the treatment available is D50.

Proper assessment of pulses, temperature and color.

This will help in monitoring neuropathy.

Reporting the BP which is more than 160 mm hg.

If the BP is controlled, it will help to prevent diseases relating coronary artery, stroke and nephropathy.

Proper monitoring of urine albumin in order to serum creatinine.

In case of renal failure, creatinine is 1.5 mg/dl. Thus, suitable measures need to be taken for the same.

 

In this assessment, we have considered one example. It is about the patient who is suffering from Diabetes 2. It is one of the severe diseases. In such a disease, injecting insulin is the only solution (Candace. 2009). The patient needs to maintain complete diet in order to keep his diabetes under control. He should not intake sugar in his food, being indigenous, he should have a control on his food, he should try to intake his food on time. This will make everything to work out systematically and thereby, helps him to recover on time.

The same patient is suffering from the problems such as, slurred speech, mobilization with an unsteady gait and with blood pressure. There is a severe deficit on his right side. This is due to his age and also due to the factors that are genetic or hereditary by nature. It may happen that already one of his family members were suffering from such disease and so on. Thus, in such a case, a patient needs to maintain a regular diet in order to live a normal life. The BP of this patient is abnormally high. In such a case, this patient needs to maintain high diet where he has to eat food containing minimum salt in it and should go for regular check up. This will help him to know the real status which will be a kind of precaution actually taken by him.

Conclusion-

Thus, it can be concluded that the adoption of various technology within the hospital also plays an important role to provide quality services to its patient. It will help the patient to attract towards the hospital. This will also help in enhancing the overall performance of the hospital. The skills and the nature of the staff also plays an important role in curing the patient. If the nursing staff is skillful enough, cooperative and friendly by nature, it will help the patients to feel happy and to go for the treatment happily. Proper treatment will help the patient to cure their diseases immediately (Alan. 1999). It will also help the patient to know about the services of the hospital as a whole. This will help in the overall growth of the hospital. Proper planning and recording play an important role in this whole aspect. Thus, need to be focused on. The well qualified doctors are also playing a vital role in this whole aspect. The services rendered by the qualified doctors will help to keep the patients satisfactory and happy. This will enhance the total performance.

After the quality of services is improved, it will help them to do or attempt continuous efforts in order to keep the patient happy with their services. The staff should not be rude in their tone as this may create a bad impression and should be avoided strictly (S QAMAR, 1990). If anything comes as a challenge in front of the staff of a hospital, the same can be considered by the hospital as a great opportunity for them. There should be coordination which will make the staff to cure the patient on time. In case of a patient Jimmy, the medication recommended are Metformin, Glipizide, Clopidogrel 75 mg and Atorvastatin 20 mg. Thus, they should be continued till there is a recovery in his condition. Once he gets recovered, the dosage will be reduced automatically. But this medicine will be continued till he recovers immediately. The recovery time period can be more than six months. Thus, the condition of the patient will get improved only with the help of intake of medication on time. This will help them to deal with the diseases strongly and will help to improve their resistance power. Thus, helpful in a great manner (Irvine, 2016) .
